3、Qualification Requirements For Bidder
Qualifications or Performance:1. the tenderer shall have independent legal personality;
2. if the bidder is a manufacturer, it shall have a medical device production license (except for scientific research equipment); If the bidder is an agent, it shall provide a valid business license or business Filing Certificate for medical devices (if necessary) (except for scientific research equipment) in accordance with the provisions of the measures for the supervision and administration of medical devices (Order No. 8 of the State Food and Drug Administration);
3. the bidder shall provide the medical device registration certificate (if any, the attached table) or product filing form (if necessary) (except for scientific research equipment) of the invested equipment in accordance with the measures for the administration of medical device registration (Order No. 4 of the State Food and Drug Administration);
4. if the bidding medical research equipment belongs to the catalogue of measuring instruments (type approval part) managed by the state according to law, the type approval certificate of measuring instruments shall be provided; If the bidding medical scientific research equipment is a disinfection product, the health and safety evaluation report of the disinfection product or the health license approval document (if necessary) must be provided;
5. the bidder is the manufacturer or agent of the product, the agent bidding shall have the letter of authorization or agent certificate issued by the manufacturer, and only one agent of the same brand is allowed to participate in the bidding;
6. the bidder who has accumulated more than two invalid queries in the past six months and more than three invalid queries in the past year, as determined by the competent department, is not eligible for bidding;
